  1. All cases opened by the NHSCFA between 2018 to 2021 relating to payment by results fraud indicating the organisation/Trust impacted, the outcome of the investigation (ie closed, ongoing, dismissed), the amount of money impacted and recovered and, where applicable, the number of sanctions.
  2. All cases opened by the NHSCFA between 2018 to 2021 relating to up-coding/over-coding fraud indicating the organisation/Trust impacted, the outcome of the investigation (ie closed, ongoing, dismissed), the amount of money impacted and recovered and, where applicable, the number of sanctions.
  3. All cases opened by the NHSCFA between 2018 to 2021 relating to perverse incentives/abuse of a payment scheme indicating the organisation/Trust impacted, the outcome of the investigation (ie closed, ongoing, dismissed) ,the amount of money impacted and recovered and, where applicable, the number of sanctions.
