Information requested
I am writing to request data on fraud cases reported by individual NHS Trusts during 2018-2025. I would like the dataset to be provided as an Excel spreadsheet with the following information: (1) the total number of cases per Trust-year; (2) a breakdown (as detailed as possible) of reports by fraud type per Trust-year; (3) action taken; and (4) investigation outcome and total losses if available.
I would like the data to contain all fraud cases reported to NHSCFA, irrespective of the source, broken down by individual NHS Trusts

Attached is a spreadsheet containing the following information:
- The total number of fraud cases reported to NHSCFA for the period 2021-2025.
- The total number of cases by taxonomy category, per Trust.
- The total number of investigations for each Trust.
- I am unable to provide information for action taken, investigation outcome and total losses for each Trust.
For the category of ‘action taken’ a report would be recorded as ‘closed’ by NHS Counter Fraud Authority or disseminated to the Trust.
Therefore, for a record of action taken, it would be necessary to submit a separate request to each individual Trust for the information.
NHS Counter Fraud Authority does not hold information for the total losses for each of the Trusts.
